Travel Authorization for Minors
Requirements

ID card of the requesting father and/or mother (if Dominican)

Valid passport of the requesting father and/or mother (if foreign)
3
Copy of the ID of the authorizing father and/or mother
State ID, state driver's license

Copy of the ID card or passport (as applicable) of the adult accompanying the minor
5
Copy of the minor's valid passport
6
Copy of the minor's birth certificate
Notes
- If the minor is traveling without either parent, the permit must be requested by each parent in the country where they are located.
- The travel authorization for the minor must be legalized online through the following link: Legalize document
- After the document has been legalized, it must be taken to the Immigration Office of the Dominican Republic so that the minor can leave the country.
