Route Letter
Requirements for Adults
Copy of both sides of the applicant's Dominican Identity and Electoral Card.
Issued from 2020 onward, with QR code.
Copy of the Police Report certification or Passport Loss Affidavit (if applicable), prepared at the Consulate, in case of a lost or stolen passport.
Copy of state ID or driver's license.
With white background, facing forward, without gestures, ears uncovered, without glasses, without chains, without necklaces, without earrings, and without low-cut neckline.
Requirements for Minors
Copy of both sides of the Dominican Identity and Electoral Card of both parents.
Copy of state ID or driver's license.
Completed at the Consulate and signed by the father or mother before the Consul or Vice Consul, explaining the reason for the Route Letter request or the missing document, passport booklet. This document is free of charge.
In addition, you must present the same requirements for the Route Letter for adults: copy of the minor's expired passport, recent birth certificate of the minor, recent 2x2 photo, and flight itinerary or airline ticket.
Process
- Send the color-scanned documents, in PDF format, to the email address: crpm.consuladordchicago@gmail.com.
- Attend the Consulate, by appointment, to complete the Affidavit (if it is due to passport loss or if it involves a minor) and make the payment for this service (if applicable).
- Attend the Consulate, by appointment, to pick up the Route Letter and make the payment.
