Travel Authorization for Minors
Requirements

ID card of the requesting father and/or mother

Valid passport of the requesting father and/or mother
3
Copy of the ID of the authorizing father and/or mother
State ID, state driver's license

Copy of the passport of the adult accompanying the minor
5
Copy of the minor's valid passport
6
Copy of the minor's birth certificate
Notes
- If the minor (boy or girl) is traveling without the parent who is requesting this permit at the Consulate, that parent must apply for the minor's permit in the country where they are located.
- The travel authorization for the minor must be legalized online through the following link: Legalize document
- After the document has been legalized, it must be taken to the Immigration Office of the Dominican Republic so the minor can leave the country.
