Emergency Travel Document

For Adults

• Dominican Identity and Electoral Card (expired in 2024)
• Copy of the most recent passport or loss report
• Birth Certificate (issued from 2020 onwards, with QR code on the lower right margin)
• Copy of state ID or driver’s license (both valid) / proof of address
• Recent 2 x 2 photo (white background, facing forward, no facial expressions, no glasses, no necklace, no earrings, no piercings)
• Flight reservation or airline ticket

Price: 50 dollars via money order payable to the Dominican Consulate

Processing time: 3 to 5 business days

For Minors

• Include a copy of the Dominican Identity and Electoral Card of both parents
• ID of the applying parent (state ID or driver’s license)
• Both father and mother must send a letter authorizing the Consulate, which can be handwritten or typed
• Minor’s birth certificate (issued from 2020 onwards, with QR code on the lower right margin)
• Application Form

Price: 50 dollars via money order payable to the Dominican Consulate

• You may send scanned color documents in PDF format to the following email address: crpm.consuladordchicago@gmail.com or send them by postal mail to the physical address of the Dominican Consulate: 8770 W. Bryn Mawr Ave. St. 1300. Chicago, IL 60631
• If you wish to receive the Roadmap Letter by postal mail, you must send a prepaid envelope with postage and tracking number for the return of the document. If you proceed by postal mail, the time to receive your letter will depend on the value/time of the postage you purchase (one day must be added to that time).
