Copy of state ID or driver's license, both valid, or proof of address.

Recent 2 x 2 photo
White background, facing forward, without gestures, without glasses, without necklaces, without earrings, and without piercings.

Flight reservation or airline ticket

State ID or driver's license.

Notes
- You may send color-scanned documents in PDF format to the following email address: crpm.consuladordchicago@gmail.com, or you may send them by postal mail to the physical address of the Dominican Consulate: 8770 W. Bryn Mawr Ave. St. 1300, Chicago, IL 60631.
- If you wish to receive the travel letter by postal mail, you must send a prepaid envelope with postage and tracking number for document return, as well as the Money Order. If you complete the process by postal mail, the time to receive your letter will depend on the postage you purchase; you must add 1 day to that time.
