It must be duly apostilled.

Prepaid envelope
You must include stamps, your address for the return of the documents, and tracking number.

Personal information
Include a sheet of paper in the envelope with your full name, phone number, and email address.

Mailing
Send everything to our address: Dominican Consulate, 8770 W Bryn Mawr Ave., Ste 1300, Chicago, IL 60631.
Notes
- Any document issued by US authorities must be apostilled before requesting the translation. The Apostille is obtained through the Secretary of State of the state that issues the document. You must communicate with them directly.
