Last Name Change by Marriage or Divorce in the USA

Requirements

For data correction, marital status and/or married or single last name, if you completed the process in the United States, the steps are as follows:

  • Transcribe the certificate
  • Correct the national ID

Correct ID

Request the national ID data change

Through the “SCHEDULE ID APPOINTMENT” button, once the minutes/certificate transcription has been completed (see related pages at the end).

Copy of the Dominican birth certificate

Must be legalized. If you do not have it, you must request it on the day you complete the transcription process.

Copy of the already transcribed marriage or divorce certificate

Must be in Dominican format.

Valid ID

Must include your photo.
