- Death certificate (original)
- Burial transfer permit (original)
- Certificate of non-communicable disease (original)
- Cremation certificate or embalming certificate (original)
- Copy of the deceased's passport
- Prepaid envelope with address and postage for returning documents

Send the documents by mail to our Dominican Consulate address:

Fee
250 dollars by money order / certified check (commercial checks from the Funeral Home will be accepted) payable to the Dominican Consulate
Processing time
5 to 7 business days once the documents are received at our office.Notes
- Each individual document requires an Apostille from the Secretary of State issuing the document (not required for passport photocopy).
- Apostilles must be obtained before sending the documents to the consulate.
- Incomplete files will delay the process.
IMPORTANT: All bodies must be sent in a hermetically sealed coffin.
