Transfer of Human Remains

Requirements

  • Death certificate (original)
  • Burial transfer permit (original)
  • Certificate of non-communicable disease (original)
  • Cremation certificate or embalming certificate (original)
  • Copy of the deceased's passport
  • Prepaid envelope with address and postage for returning documents
  • Send the documents by mail to our Dominican Consulate address:
    8770 W Bryn Mawr Ave., Ste 1300, Chicago, IL 60631

Fee

250 dollars by money order / certified check (commercial checks from the Funeral Home will be accepted) payable to the Dominican Consulate

Processing time

5 to 7 business days once the documents are received at our office.

Notes

  • Each individual document requires an Apostille from the Secretary of State issuing the document (not required for passport photocopy).
  • Apostilles must be obtained before sending the documents to the consulate.
  • Incomplete files will delay the process.

IMPORTANT: All bodies must be sent in a hermetically sealed coffin.
