Requirements
- Required affidavit template. In case of a financial procedure, the financial institution must provide the document template.
- Identity and Electoral Card of the declarant(s).
- Copy of ID with updated address or proof of residence (bills or official letters).
- Precise information about the purpose of the affidavit.
- Copy of the Dominican witness’s ID who cannot be a relative.

Process

- In-person signature at the consulate is mandatory.
- Bring the Money Order on the day you come to the consulate to sign.
Once you complete your application, we will coordinate with you to schedule a convenient day for you to sign and collect your document.
