Requirements
- ID card of the applying parent(s)
- Passport of the applying parent(s)
- Copy of the authorizing parent’s ID(s) (state ID, state driver’s license)
- Copy of the accompanying adult’s passport
- Copy of the minor’s passport
- Copy of the minor’s birth certificate
Fee: 50 dollars via money order payable to the Dominican Consulate
Notes
- The applying parent must come to the consulate to sign the permission for the minor.
- If the minor (boy or girl) travels without the parent applying for this permission at the Consulate, the permission must be requested in the country where the minor is located.
- The minor’s permission must be sent to the Dominican Republic and, once there, the document must be taken to MIREX to the legalization department.
- After the document has been legalized, it must be taken to Immigration so the minor can leave the country.
